This requires an additional Survey Form. Aka as Single Form Surveys. This is a survey-form that has the full (all 3 (or 5) faces) icons. The user can make their selection in the form. NOTE: The short URL can be used in a SMS to the client (E.g. customers who use SMS to notify and close off tickets)
